Job summary

Senior Community Care of Michigan PACE, a program of Volunteers of America National Services, is seeking a Social Worker in Lansing, MI. The role focuses on enrollment assessments, care planning, and ongoing support for seniors in a PACE setting.

Qualifying candidates hold a Master’s in Social Work, licensure in the state, and at least one year of elderly care experience, plus a valid driver's license and transportation. Pay ranges from $60,000–$80,000 depending on LMSW/LLMSW status and schedule.

Qualifications

• Masters’ Degree from an accredited school of social work.
• Be legally authorized, currently licensed, registered or certified if applicable in the state of employment.
• A minimum of one year’s experience working with frail or elderly population required.
• Must have a valid driver’s license and have means of transportation.

Responsibilities

• Performs in person initial assessments for enrollment of potential Senior Community Care participants to obtain a complete psychosocial history.
• Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant and conducts re-assessments every six months.
• Maintains regular attendance at Interdisciplinary Team meetings and communicates participant changes for 24-hour care delivery.
• Provides ongoing support, counsel, and education to participants and family regarding aging, dementia, grief and life changing issues.
