Job summary

Scripps Health is seeking a Liver Transplant Coordinator to manage comprehensive pre- and post-transplant patient care at Scripps Green Hospital – Torrey Pines. You will work with a multidisciplinary team to ensure seamless transitions from inpatient to outpatient settings and support transplant readiness.

The role requires a BSN and 3 years of clinical nursing experience, with current BLS certification. Preferred MSN and transplant-specific experience are a plus.

Qualifications

• BSN required.
• 3 years of recent clinical nursing experience.
• Current BLS certification from American Heart Association.

Responsibilities

• Develops individualized transplant care plans based on assessments and protocols.
• Coordinates multidisciplinary services across the transplant continuum (evaluation, waitlist, hospitalization, discharge, follow-up).
• Educates patients and caregivers on liver disease, transplantation, immunosuppression, and post-transplant care.
• Acts as patient advocate, ensuring timely communication and continuity of care.
