Job summary

Cleveland Clinic Weston Hospital is seeking a Liver Transplant Coordinator to oversee liver transplant patient activities and coordinate care plans with the transplant physician and team.

The role requires strong RN experience, coordination abilities, and collaboration with inpatient and outpatient caregivers. Typical schedule is days with on-call coverage, supporting a high-quality patient experience.

Qualifications

• RN experience (two years minimum) in inpatient/outpatient settings.
• BLS certification through AHA/Red Cross.
• Bachelor's Degree preferred; transplant or GI experience is a plus.

Responsibilities

• Assess patients in outpatient clinic with transplant physician.
• Facilitate care plans and symptom management with outpatient caregivers.
• Coordinate follow-up appointments and scheduling with administrative staff.
• Review medical information from referring physicians and initiate communication.
• Coordinate with transplant team for evaluation, tissue typing, and candidacy.
