Job summary

Access Services is looking for a Lifesharing Coordinator to connect individuals with intellectual disabilities to appropriate provider homes. This role involves coordinating person-centered plans and working closely with families and community partners to enhance clients’ integration into community life.

Qualifications

• Experience supporting individuals with intellectual and developmental disabilities.
• Valid U.S. driver's license and reliable access to a personal vehicle.
• Knowledge of behavior management and safety techniques.

Responsibilities

• Coordinate and oversee person-centered plans for individuals.
• Match individuals with appropriate Lifesharing provider homes.
• Train and support providers on individual service plans.
• Facilitate collaboration with community partners and organizations.

Skills

Strong organizational, communication, and relationship-building skills

Ability to collaborate with individuals, families, providers, and public partners

Knowledge of person-centered planning and community-based supports

Ability to balance office work, community-based work, and on-call responsibilities

Education

Bachelor’s degree plus 2 years of experience supporting individuals with IDD

Associate degree/60 credits plus 4 years of experience supporting individuals with IDD

High school diploma plus 6 years of experience supporting individuals with IDD
