Job summary

Access Services seeks a Lifesharing Coordinator to connect individuals with Intellectual and Developmental Disabilities to Lifesharing provider homes that best match their needs and goals.

You will collaborate with individuals, families, providers, and community partners to plan, coordinate services, and promote dignity and independence in home and community life. The role includes on-call rotation and requires travel across the Delaware Valley with a primary Fort Washington, PA office location.

Qualifications

• Bachelor's/Associate/High school pathway with relevant IDD experience.
• Background checks, physical exam, and driving history required.
• Ability to work in hybrid office-site roles and travel regionally.

Responsibilities

• Coordinate person-centered plans addressing social, emotional, physical, and educational needs.
• Match individuals with Lifesharing provider homes to support transitions.
• Train and support providers on service plans and regulatory requirements.
• Monitor provider homes for safety and regulatory compliance.
• Facilitate meetings and collaborate with SCOs, AEs, and community partners.
• Ensure timely documentation including notes, assessments, and reviews.
• Connect individuals and families with community resources and respite services.
• Participate in on-call coverage and after-hours support.
