Job summary

Kentucky River Community Care in Jackson, KY is seeking a Life Skills Coach/Community Support Associate to assist individuals with behavioral health services. Responsibilities include teaching skills to access resources, documenting in EMR within 24 hours, and promoting independence in the community while following treatment plans.

Monday–Friday, 8:00 AM–4:00 PM; HS diploma or GED and a valid driver’s license with driving experience required.

Qualifications

High School diploma or GED with one year of full-time experience working with individuals who receive services for treatment of a behavioral health disorder.
Valid driver's license with 2 years of driving experience.

Responsibilities

Teaching clients the following skills: how to utilize/access community resources, medications adherence, interpersonal skills, behavioral support skills, crisis coping skills, emotional regulation skills, how to access transportation, how to budget/manage finances, and how to shop independently in the community.
Complete documentation in EMR within 24 hours of service delivery.
Conduct activities necessary to allow individuals to live with maximum independence in the community.
Enhance community living through utilizing skills training, cueing or supervision as identified in recipients treatment plan.
Maintain all trainings and required supervision.
Participate in therapeutic rehabilitation and 1915i Rise services as needed.
