Job summary

Kentucky River Community Care in Hazard, KY is seeking a Life Skills Coach/Community Support Associate to support individuals with behavioral health needs. The role requires a High School diploma or GED and experience with behavioral health services, plus a valid driver’s license.

The position is a Regular Full-Time role with daytime hours, focusing on skills training, community integration and independent living supports.

Qualifications

• High School diploma or GED with one year of full-time experience working with individuals who receive services for behavioral health disorders (required).
• Must have valid driver’s license with 2 years of driving experience (required).

Responsibilities

• Teach clients to access community resources, manage medications adherence and develop interpersonal, crisis coping, emotional regulation, transportation, budgeting and independent shopping skills.
• Complete EMR documentation within 24 hours of service delivery.
• Conduct activities to enable individuals to live with maximum independence in the community.
• Enhance community living through skills training, cueing or supervision per the treatment plan.
• Maintain all trainings and required supervision.
• Participate in therapeutic rehabilitation and 1915i Rise services as needed.

Primary functions:

• Teaching clients the following skills: how to utilize/access community resources, medications adherence, interpersonal skills, behavioral support skills, crisis coping skills, emotional regulation skills, how to access transportation, how to budget/manage finances, and how to shop independently in the community.
• Complete documentation in EMR within 24 hours of service delivery.
• Conduct activities necessary to allow individuals to live with maximum independence in the community.

• Maintain all trainings and required supervision.

Shift/Days & Work Hours: Monday - Friday 8:00AM - 4:00PM

Funding Source: Meet requirements 908 KAR 2250, Billable for H2015, Medicaid.
