Job summary

Horizon House, Inc. is seeking a full-time Family Living Coordinator / Life Sharing Coordinator in Philadelphia, PA.

The role focuses on overseeing the Life Sharing program, coordinating assessments and service plans, and building supportive relationships with individuals and families within developmental services. The position requires a master’s degree (or BA with MR experience), strong confidentiality, organizational and communication skills, and willingness to work flexible hours including

Qualifications

• MA in Human Services/Social Sciences/Healthcare or related field with 1+ year MR experience, or BA with 2+ years MR experience (or equivalent).
• Knowledge of family dynamics and confidentiality of client and staff records.
• Strong analytical, organizational, and communication abilities.
• Experience writing programming goals; ability to work independently and stay detail-oriented.

Responsibilities

• Oversee and monitor Life Sharing program.
• Participate in interviewing and selecting prospective families.
• Coordinate and implement assessments and service plans with individuals and families per 6500 regulations.
• Support relationships within the family.
• Develop relationships with individuals interested in Life Sharing.
• Be available for flexible work schedule, including evenings and weekends.
• May participate in investigations and on-call coverage as needed.
