Job summary

ADAPT Community Network in New York City is seeking a dedicated Case Manager to develop, implement, and monitor Life Plans for residents in our housing settings. Under the supervision of the Program Director, you will advocate for individuals, coordinate services, and engage families while promoting dignity and independence.

Qualifications

• Bachelor’s Degree in Human Services or related field required.
• Minimum of one (1) year of experience working with individuals with developmental disabilities required.
• Ability to read, write, speak, and understand English.
• Strong interpersonal and communication skills with the ability to interact effectively with people supported, families, staff, and outside agencies.

Responsibilities

• Provides social services to individuals we support and works toward maximizing their social functioning.
• Develops, monitors, and updates Individualized Service Plans.
• Participates in and chairs interdisciplinary team meetings and follows up on recommendations.
• Collaborates with residential staff, clinical staff, and service providers to ensure coordinated support.
• Identifies aptitudes and preferences and develops person-centered goals and activities.
• Maintains accurate, timely documentation including monthly case notes and family contacts.
