Job summary

ADAPT Community Network in Louisiana is seeking a Case Manager to develop, implement, and monitor Life Plans for people we support in residential settings. Under supervision of the Program Director, you will ensure services align with regulatory standards and personal goals, while promoting dignity, independence, and quality of life.

You will advocate for rights, coordinate services with families and external partners, maintain documentation, and lead interdisciplinary team meetings.

Qualifications

• Bachelor’s Degree in Human Services or related field.
• Minimum of one year of experience with individuals with developmental disabilities.
• Ability to read, write, speak, and understand English.
• Strong interpersonal and communication skills.
• Ability to work independently, as part of a team, and with flexible scheduling.

Responsibilities

• Develops, monitors, and updates Individualized Service Plans.
• Chairs interdisciplinary team meetings; documents and follows up on recommendations.
• Coordinates admissions, referrals, and day program arrangements.
• Advocates for rights and ensures benefits are applied for in a timely manner.
• Maintains accurate monthly case notes, progress updates, and family contacts.
