UMass Memorial Health seeks a skilled social worker to deliver psychosocial assessments and treatment planning within a multidisciplinary team. You will conduct intake interviews, coordinate daily team meetings, and provide individual, couple, and family therapies as part of comprehensive care.

The role requires an MSW, LICSW licensure, and 1–3 years of social work experience, with strong communication and data skills.

Formación

• Masters degree in Social Work.
• Current Massachusetts Licensed Clinical Social Worker (LICSW) licensure.
• One to three years of social work experience.
• Demonstrated ability in psychosocial assessment, knowledge of community resources, crisis intervention skills, ability in discharge planning, and ability to work with a multidisciplinary team.
• Data compilation and analysis skills.
• Excellent written and communications skills.
• Basic computer skills.

Responsabilidades

• Conducts intake interviews with patients and families to develop psychosocial evaluations.
• Participates in team planning of treatment during initial interview and coordinates team meeting on a daily basis.
• Provides psycho-social assessments and treatments, individual and group education/therapy to include dual diagnosis cases and all associated social work services.
• Serves as a primary clinician for individual therapy, couples and family therapy.
• Works with outpatient providers to coordinate treatment planning.
