Key responsibilities include:

• Verify Plan of Care and provide care according to delegation, orders, and standards.
• Incorporate patient care goals into visits, documented appropriately.
• Communicate significant findings and changes to the team.
• Report unsafe conditions and implement safety plans.
• Assist patients in self-care techniques and suggest care plan revisions.
• Deliver services requiring specialized nursing skills under supervision.
• Collaborate with RN Case Manager for advice and instructions.
• Document medication regimens and update profiles.
• Treat patients and caregivers effectively.
• Prevent avoidable hospitalizations and ER visits.
• Communicate positively with all parties involved in care.

Minimum requirements:

• Graduate from an accredited nursing program.
• Current State License as LVN/LPN.
• At least 1 year of clinical care experience.
• Experience in hospice or home health preferred.

• 50% travel required.

• Physical ability to lift 50-100 lbs, sit, stand, bend, and move intermittently.

• Valid driver’s license, insurance, and reliable transportation.

Reporting to the Clinical Team Manager.
