Job summary

Elara Caring is seeking a Licensed Practical/Vocational Nurse (LPN/LVN) to provide one-on-one home health care. You will work closely with an RN Case Manager to follow individualized care plans, document medications, and monitor patient progress in patients' homes.

The role requires a current state license, graduation from an accredited nursing program, and 1 year of clinical experience; home health or hospice experience is preferred.

Qualifications

• Current State License as a Licensed Vocational / Practical Nurse (LPN/LVN)
• Graduate of an accredited Certificate, Diploma, or Associate School of Nursing
• 1 year of experience in a clinical care setting
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment
• Experience in a hospice or home health environment is preferred
