Position Details

• Location: San Diego, CA (Must live in California & have remote work experience)
• Compensation: $65,000 - $85,000 annually

Role Overview

We are seeking an experienced medical professional with a passion for delivering high-quality, compassionate care. The LVN collaborates with the interdisciplinary care team to coordinate care for complex patients, requiring critical thinking and excellent organizational skills. We focus on engaging and caring for medically complex patients with empathy and a person-centered approach.

Responsibilities

1. Perform assessment, treatment planning, and ongoing care management using a person-centered approach.
2. Make and facilitate referrals for social services, mental health, medical, and substance abuse care.
3. Use motivational interviewing to engage and educate patients and coordinate follow-up care.
4. Update registries, care plans, and clinical notes.
5. Participate in care team meetings.

Qualifications

• 2+ years of experience as an LVN
• Unencumbered LVN license
• Bilingual skills preferred
• Remote work experience required
