Job summary

Sweetwater Care in California is seeking an LVN to provide nursing care within scope of practice, following physician orders, center policies, and all applicable guidelines. The role requires California licensure and experience in long-term care.

The LVN will participate in quality initiatives, educate patients and families, and ensure timely administration of medications and treatments. Knowledge of Point Click Care is highly valued.

Qualifications

• California-licensed LVN is required.
• 1 year nursing management experience preferred.
• Long-term care experience is required.
• Point Click Care charting experience is required.

Responsibilities

• Demonstrates knowledge and appropriate use of the Infection Control Manual.
• Participates in and assists with the Quality Improvement Program.
• Uses the Quality Life Manual for patient placement and monitoring.
• Notifies departments of patient changes (room changes, LOA, admits, discharge).
• Educates patients and families.
• Administers medications on schedule per facility policy.
• Administers treatments on schedule per facility policy.
• Receives, transcribes and executes physician orders as allowed by Nurse Practice Act.
• M conducts rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state of California
• One-year prior nursing management experience preferred, not required.
