Job summary

Sweetwater Care in Kingsburg, California is seeking an LVN to provide nursing care within scope of practice, following physician orders, center policies and applicable guidelines.

The role requires an active California LVN license, LTC experience, and familiarity with Point Click Care charting; management experience is a plus. The successful candidate will collaborate with the care team and participate in quality improvement activities.

Qualifications

• Must hold an active LVN license in California.
• Prior nursing management experience is preferred but not required.
• Experience in long-term care settings is required.
• Familiarity with Point Click Care charting is preferred.

Responsibilities

• Provide nursing care within scope of practice per standards and physician orders.
• Participate in and assist with Quality Improvement activities.
• Utilize the Quality Life Manual for patient placement and monitoring.
• Notify other departments of patient changes (room changes, admissions, discharges).
• Conduct family and patient education.
• Administer medications in a timely manner according to policy.
• Administer treatments in a timely manner according to policy.
• Receive, transcribe, and carry out physician orders as allowed by state regulations.
• Take rounds with physicians when necessary.

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state of California
• One-year prior nursing management experience preferred, not required.
