Job summary

Sweetwater Care seeks an LVN with leadership skills to provide nursing care in a long-term care setting, following physician orders, center policies, and state guidelines. The role involves medication administration, treatments, family education, and coordinated rounds with physicians.

Experience in long-term care and PCC charting is preferred. Licensing must be current in California, with a willingness to work within our Quality Improvement and Infection Control frameworks.

Qualifications

• Currently licensed as LVN in California
• One-year nursing management experience preferred, not required
• Long term care experience
• PointClickCare charting experience

Responsibilities

• Provides nursing care within scope of practice to meet patient needs per standards, physician orders, and policies
• Notifies departments of patient changes such as room changes, admissions, or discharges
• Conducts family and patient education
• Administers medications timely and per facility policy
• Administers treatments timely and per facility policy
• Receives, transcribes, and carries out physician orders when permitted by Nurse Practice Act
• Makes rounds with physicians as needed

Job description

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state of California
• One-year prior nursing management experience preferred, not required.
