Shadelands Post Acute in Walnut Creek, CA is seeking a dedicated Licensed Vocational Nurse (LVN) to join our clinical team on a part-time basis. The role focuses on delivering skilled nursing care with compassion, supporting resident comfort and safety in a post-acute setting.

Responsibilities include medication administration, assessments, care planning, and documentation, with supervised CNAs and collaboration with an interdisciplinary team. California LVN license and BLS/CPR are required.

Key Responsibilities

Clinical Care

• Provide skilled nursing care including medication administration, treatments, wound care, and monitoring vital signs
• Deliver compassionate, resident-centered care focused on comfort, dignity, and safety

Assessment & Monitoring

• Observe, assess, and document changes in resident condition
• Communicate concerns promptly to the RN Supervisor and healthcare providers

Care Planning

• Assist with the development and implementation of individualized care plans
• Support resident recovery goals and quality-of-life initiatives

Leadership & Team Support

• Supervise and support Certified Nursing Assistants (CNAs)
• Collaborate with interdisciplinary team members to ensure continuity of care

Documentation & Compliance

• Maintain accurate clinical documentation and medical records
• Ensure compliance with all state, federal, and facility regulations and standards

Qualifications

• Current and active California LVN license required
• Current BLS/CPR certification required
• Previous skilled nursing or post-acute care experience preferred
• Strong clinical judgment and communication skills
• Compassionate, dependable, and team-oriented approach to resident care
