Job summary

Shadelands Post Acute in Walnut Creek, CA is seeking a dedicated Licensed Vocational Nurse (LVN) to join our clinical team on a part-time basis. The role focuses on delivering skilled nursing care with compassion, supporting resident comfort and safety in a post-acute setting.

Responsibilities include medication administration, assessments, care planning, and documentation, with supervised CNAs and collaboration with an interdisciplinary team. California LVN license and BLS/CPR are required.

Qualifications

• Current and active California LVN license required.
• Current BLS/CPR certification required.
• Experience in skilled nursing or post-acute care preferred.

Responsibilities

• Provide skilled nursing care including medication administration, treatments, wound care, and monitoring vital signs.
• Observe, assess, and document changes in resident condition; communicate concerns promptly.
• Develop and implement individualized care plans; support recovery goals.
• Supervise CNAs and collaborate with interdisciplinary team.
• Maintain accurate medical records and ensure regulatory compliance.
