Sweetwater Care in California seeks an experienced nursing supervisor to provide comprehensive care to residents in accordance with physician orders and facility policies. The role includes education for patients and families, medication/treatment administration, and collaboration with physicians.

Candidates should have an LVN license for California, with a preference for 1+ year nursing management experience and long-term care familiarity.

Qualifications

• Licensed LVN in California (or compact licensure for other states)
• 1+ year nursing management experience preferred
• Long-term care experience required
• Experience in PointClickCare charting and documentation

Responsabilités

• Provide nursing care within scope of practice following standards and orders
• Participate in Quality Improvement Program and Infection Control
• Identify and monitor patients in appropriate programs per Life Manual
• Notify departments of patient status changes (admissions, discharges, room changes)
• Educate families and patients
• Administer medications and treatments timely per facility policy
• Transcribe and execute physician orders as allowed by Nurse Practice Act
• Conduct rounds with physicians when necessary

Objective

Provides nursing care within scope of practice to ensure patient’s needs are met in accordance with standards of practice, physician orders, center policies and procedures, and state, federal and local guidelines.

Principal Responsibilities

• Demonstrates knowledge and appropriate use of the Company Infection Control Manual.
• Understands, participates and assists in implementing the Quality Improvement Program.
• Utilizes the Quality Life Manual for identification, placement, and monitoring of patients in the appropriate programs.
• Notifies other Departments of patient changes, i.e., room changes, LOA, new admits, discharge, etc.
• Conducts family and patient education.
• Demonstrates the ability to administer medications timely and according to facility policy.
• Demonstrates the ability to administer treatments timely and according to facility policy.
• Demonstrates ability to receive, transcribe, and carry out physician orders, if allowed by Nurse Practice Act
• Makes rounds with physicians when necessary.

Qualifications

• Currently licensed as LVN in state California, if contract is in the state of Califonia. Have a compact license if working in a Compact state.
• One-year prior nursing management experience preferred
• LONG TERM CARE EXPERIENCE
• POINT CLICK CARE CHARTING EXPERIENCE
