Cibolo Creek is seeking a Licensed Vocational Nurse (LVN) to join our team in Boerne, Texas. The role supports resident care in the evening shift, with responsibilities for administering medications, charting, and reporting patient status to the supervising nurse.

Qualifications

• LVN license in good standing with the State of Texas.
• Minimum 1 year of LVN experience preferred in a clinical setting.
• EMR experience (PCC preferred) and reliable transportation are required.

Responsibilities

• Follow nursing standards and community policies and procedures.
• Conduct resident rounds daily and report issues to supervisor.
• Administer and document patient care, medications, and treatments as ordered.
• Write detailed reports and maintain precise charting of vital signs and symptoms.

Responsibilities

• Follow established standards of nursing practices and implement community programs, policies and procedures.
• Conduct resident rounds daily, reporting problems to supervisor and initiating corrective action.
• Administer and document direct resident care, medications and treatments per physicians’ orders and accurately record all care provided.
• Write detailed reports, maintaining precise charting, recording medications, observing symptoms, and taking vital signs.

Qualifications

• San Antonio, TX: Reliably commute or planning to relocate before starting work (Required)
• Licensed Vocational Nurse (LVN) in good standing and currently licensed by the State of Texas.
• Minimum 1 year experience as an LVN in a clinical setting preferred.
• Electronic Medical Record Experience required, PCC preferred.
• Excellent interpersonal skills.
• Reliable transportation.
