Sharecare, Inc. via CareLinx is recruiting Licensed Vocational Nurses (LVNs) or Licensed Practical Nurses (LPNs) in Upland, CA to provide skilled nursing care across multiple settings. Responsibilities

• Provide care within your scope of practice including creating, implementing, and evaluating patient care plans.
• Collect, record, and monitor patient data, vital signs and routine measurements.
• Administer immunizations and therapeutic injections as ordered by providers.
• Supervise ancillary personnel and delegate responsibilities as appropriate.
• Ensure patient privacy, dignity, and safety in a secure environment.

WHAT YOU'LL DO

Core Responsibilities:

• Provide care within your scope of practice including preparation, implementation, and evaluation of patient care plans
• Collect, record, and monitor patient data, routine measurements, and vital signs
• Administer immunizations and therapeutic injections as ordered by providers
• Supervise ancillary personnel and delegate responsibilities as appropriate
• Ensure patient privacy, dignity, and well-being in a safe and secure environment
• Document all care tasks accurately in theCareLinxmobile app during each shift
• Submit timesheets on time through theCareLinxapp
• Communicate with your Care Advisorregardingany case updates or changes in patient condition
• Maintain client confidentiality at all times

• Additionaldocumentation may berequiredbased on state law or client contract
• Some training requirements may be needed prior to accepting shifts
