Job summary

Paycom - ATS is seeking an LVN Charge Nurse in Texas to oversee nursing care in a skilled facility. You will observe residents, administer medications, and ensure care plans are followed.

You’ll coordinate with physicians, document changes, and supervise aides while handling incident reports and discharges as required. The role requires an active Texas LVN license, completion of an LVN program, and prior SNF experience is preferred.

Qualifications

• Has completed an LVN program at an accredited school.
• Active Texas LVN license in good standing.
• Experience in skilled nursing preferred.

Responsibilities

• Observe, assess, document, and report resident condition changes during rounds.
• Ensure care plans are implemented as directed and administer meds as prescribed.
• Conduct rounds with physicians and document visits in clinical records.
• Perform narcotic reconciliation with incoming/outgoing shifts.
• Identify changes in resident condition and take action per policy.
• Oversee and monitor aides and initiate incident/accident reports.
• Discharge residents per facility policy; complete Nursing Assessments as needed.
• Communicate with residents' contacts about status and changes.
