Job summary

The LVN Charge Nurse at Paycom - ATS will assist in delivering high-quality care to residents in a Texas skilled nursing setting. You will observe and document changes, administer medications, and support care plan implementation with the nursing team.

Requirements include completion of an LVN program and an active Texas license in good standing; SNF experience is preferred. The role involves rounds with physicians and coordination with residents’ families for updates.

Qualifications

• Completion of LVN program from an accredited school.
• Current Texas LVN license in good standing.
• Experience in a skilled nursing facility preferred.

Responsibilities

• Observe, assess, document, and report resident condition or changes.
• Administer medications as prescribed.
• Assist in care plan implementation and rounds with physicians.
• Discharge residents per policy and complete required nursing assessments.
• Coordinate with the resident's family or point of contact for status updates.

Primary ResponsibilitiesThe LVN Charge Nurse will assist in ensuring the provision of quality care that promotes the highest practicable physical, mental and psychological well-being of each resident as determined by resident assessments and individual plans of care.Essential FunctionsDuring rounds observes, assess, documents, and reports resident condition/changesAssures implementation of care plans as directedAdministers medications as prescribedConduct rounds with physician and record visits in clinical recordsPerform narcotic reconciliation with incoming and outgoing shiftsIdentify significant changes in the condition of residents and takes necessary action per facility policies and proceduresOversees and monitors aidesInitiate and complete Incident and Accident Reports per facility policyDischarge residents per facility policyMust complete Nursing Assessment for Physician Calls prior to calling physician on any change in resident conditionCommunicate with resident’s point of contact when they ask for a status update or if there is a change in conditionOther special projects or functions can be assigned at times by the Director of Nurses and/or the AdministratorEducational/Training RequirementsHas completed an LVN program at an accredited vocational school or college.Licensing RequirementsMust be an active Licensed Vocational Nurse in the state of Texas in good standing.Experience RequirementsExperience working in a skilled nursing facility is preferred.Physical DemandsThe physical demands described here are representative but not necessary all inclusive, of those that must be met by an employee to successfully perform the essential functions of this job.
