Corpus-Christi-Nursing-and-Rehabilitation-Center is seeking an experienced LVN Charge Nurse to oversee resident care in our skilled nursing facility in Corpus Christi, Texas. In this role you will observe and assess residents, administer medications, chart changes, discharge when needed, and initiate nurse communication with physicians and families.

  • Active Texas LVN license required.
  • Completion of an LVN program from an accredited school.
  • Experience in a skilled nursing facility preferred.
  • Ability to perform nursing assessments and coordinate with physicians.
  • Ability to administer medications and reconcile narcotics.

Responsibilities

  • Observe, assess, document, and report resident condition/changes during rounds.
  • Ensure care plans are implemented as directed.
  • Administer medications as prescribed.
  • Conduct rounds with physicians and record visits in clinical records.
  • Perform narcotic reconciliation with shifts.
  • Oversee and monitor nursing aides.
  • Initiate and complete Incident and Accident Reports.
  • Discharge residents per facility policy.
  • Complete Nursing Assessment for Physician Calls before contacting physician.
  • Communicate status updates to residents' point of contact.

Primary ResponsibilitiesThe LVN Charge Nurse will assist in ensuring the provision of quality care that promotes the highest practicable physical, mental and psychological well-being of each resident as determined by resident assessments and individual plans of care.Essential FunctionsDuring rounds observes, assess, documents, and reports resident condition/changesAssures implementation of care plans as directedAdministers medications as prescribedConduct rounds with physician and record visits in clinical recordsPerform narcotic reconciliation with incoming and outgoing shiftsIdentify significant changes in the condition of residents and takes necessary action per facility policies and proceduresOversees and monitors aidesInitiate and complete Incident and Accident Reports per facility policyDischarge residents per facility policyMust complete Nursing Assessment for Physician Calls prior to calling physician on any change in resident conditionCommunicate with resident’s point of contact when they ask for a status update or if there is a change in conditionOther special projects or functions can be assigned at times by the Director of Nurses and/or the AdministratorEducational/Training RequirementsHas completed an LVN program at an accredited vocational school or college.Licensing RequirementsMust be an active Licensed Vocational Nurse in the state of Texas in good standing.Experience RequirementsExperience working in a skilled nursing facility is preferred.Physical DemandsThe physical demands described here are representative but not necessary all inclusive, of those that must be met by an employee to successfully perform the essential functions of this job.
