Job summary

Paycom - ATS seeks an LVN Charge Nurse to oversee quality resident care in a skilled nursing facility in Texas City. You will participate in rounds, administer medications, coordinate with physicians, and ensure care plans are followed.

The role requires a Texas LVN license and prior SNF experience. You will work with the Director of Nurses to maintain high standards and resident well-being.

Qualifications

• Has completed an LVN program at an accredited vocational school or college.
• Must be an active Licensed Vocational Nurse in the state of Texas in good standing.
• Experience in a skilled nursing facility is preferred.

Responsibilities

• Observe, assess, document, and report resident condition/changes during rounds.
• Administer medications as prescribed.
• Conduct rounds with physician and record visits in clinical records.
• Perform narcotic reconciliation with incoming and outgoing shifts.
• Oversee and monitor aides.
• Initiate and complete Incident and Accident Reports per facility policy.
• Discharge residents per facility policy.
• Complete Nursing Assessments for physician calls prior to contacting the physician.
• Communicate with residents' point of contact regarding status updates or changes in condition.
