Job summary

Pflugerville-Nursing-and-Rehabilitation-Cente is seeking an LVN Charge Nurse to support quality resident care in a skilled nursing setting. You will perform rounds, assess residents, document changes, and administer medications under physician orders, ensuring care plans are followed and updated as needed.

Ideal candidate holds an active Texas LVN license, has completed an LVN program, and possesses experience in a skilled nursing facility.

Qualifications

• Must have completed an LVN program at an accredited vocational school or college.
• Must hold an active Texas LVN license in good standing.
• Experience in a skilled nursing facility preferred.

Responsibilities

• Observe, assess, document, and report resident condition during rounds.
• Administer medications as prescribed and ensure care plans are implemented.
• Perform narcotic reconciliation with incoming and outgoing shifts.
• Identify changes in resident condition and take action per policies.
• Discharge residents per facility policy and complete nursing assessments as needed.
• Complete incident and accident reports and communicate with residents' contacts.

Primary ResponsibilitiesThe LVN Charge Nurse will assist in ensuring the provision of quality care that promotes the highest practicable physical, mental and psychological well-being of each resident as determined by resident assessments and individual plans of care.Essential FunctionsDuring rounds observes, assess, documents, and reports resident condition/changesAssures implementation of care plans as directedAdministers medications as prescribedConduct rounds with physician and record visits in clinical recordsPerform narcotic reconciliation with incoming and outgoing shiftsIdentify significant changes in the condition of residents and takes necessary action per facility policies and proceduresOversees and monitors aidesInitiate and complete Incident and Accident Reports per facility policyDischarge residents per facility policyMust complete Nursing Assessment for Physician Calls prior to calling physician on any change in resident conditionCommunicate with resident’s point of contact when they ask for a status update or if there is a change in conditionOther special projects or functions can be assigned at times by the Director of Nurses and/or the AdministratorEducational/Training RequirementsHas completed an LVN program at an accredited vocational school or college.Licensing RequirementsMust be an active Licensed Vocational Nurse in the state of Texas in good standing.Experience RequirementsExperience working in a skilled nursing facility is preferred.Physical DemandsThe physical demands described here are representative but not necessary all inclusive, of those that must be met by an employee to successfully perform the essential functions of this job.
