Jefferson-Nursing-&-Rehabilitation-Cente is seeking an LVN Charge Nurse to help ensure quality care that supports residents' physical, mental, and emotional well‑being in line with individualized care plans. You will round, assess, document changes, and report them, administer medications, and collaborate with physicians and families.

  • Must have completed an LVN program at an accredited vocational school or college.
  • Active Licensed Vocational Nurse license in the state of Texas in good standing.
  • Experience in a skilled nursing facility preferred.
  • Ability to observe, document, and report changes in resident conditions.
  • Strong communication with residents, families, and health care team.

  • Administer medications as prescribed and perform narcotic reconciliation.
  • Discharge residents per facility policy.
  • Complete Nursing Assessments prior to physician calls.

Job description

Primary ResponsibilitiesThe LVN Charge Nurse will assist in ensuring the provision of quality care that promotes the highest practicable physical, mental and psychological well-being of each resident as determined by resident assessments and individual plans of care.Essential FunctionsDuring rounds observes, assess, documents, and reports resident condition/changesAssures implementation of care plans as directedAdministers medications as prescribedConduct rounds with physician and record visits in clinical recordsPerform narcotic reconciliation with incoming and outgoing shiftsIdentify significant changes in the condition of residents and takes necessary action per facility policies and proceduresOversees and monitors aidesInitiate and complete Incident and Accident Reports per facility policyDischarge residents per facility policyMust complete Nursing Assessment for Physician Calls prior to calling physician on any change in resident conditionCommunicate with resident’s point of contact when they ask for a status update or if there is a change in conditionOther special projects or functions can be assigned at times by the Director of Nurses and/or the AdministratorEducational/Training RequirementsHas completed an LVN program at an accredited vocational school or college.Licensing RequirementsMust be an active Licensed Vocational Nurse in the state of Texas in good standing.Experience RequirementsExperience working in a skilled nursing facility is preferred.Physical DemandsThe physical demands described here are representative but not necessary all inclusive, of those that must be met by an employee to successfully perform the essential functions of this job.
