Job summary

Rockport Nursing and Rehabilitation Center in Texas seeks an LVN Charge Nurse to help provide high‑quality care that supports residents’ physical, mental and psychosocial well‑being, in line with assessments and individualized care plans.

You will participate in rounds, observe changes, administer prescribed medications, conduct narcotic reconciliations, supervise aides, and ensure accurate documentation and timely discharges.

Qualifications

  • Has completed an LVN program at an accredited vocational school or college.
  • Active LVN license in Texas in good standing.
  • Experience in a skilled nursing facility preferred.

Responsibilities

  • Assists in providing quality resident care based on assessments and care plans.
  • Observes, assesses, documents resident condition during rounds and reports changes.
  • Administers medications as prescribed and reconciles narcotics per policy.
  • Oversees aides and ensures care plan implementation across shifts.
  • Initiates and completes incident/accident reports as required.
  • Communicates with residents or families to provide status updates.
  • Discharge planning and collaboration with physicians and staff as needed.

Job description

Primary ResponsibilitiesThe LVN Charge Nurse will assist in ensuring the provision of quality care that promotes the highest practicable physical, mental and psychological well-being of each resident as determined by resident assessments and individual plans of care.Essential FunctionsDuring rounds observes, assess, documents, and reports resident condition/changesAssures implementation of care plans as directedAdministers medications as prescribedConduct rounds with physician and record visits in clinical recordsPerform narcotic reconciliation with incoming and outgoing shiftsIdentify significant changes in the condition of residents and takes necessary action per facility policies and proceduresOversees and monitors aidesInitiate and complete Incident and Accident Reports per facility policyDischarge residents per facility policyMust complete Nursing Assessment for Physician Calls prior to calling physician on any change in resident conditionCommunicate with resident’s point of contact when they ask for a status update or if there is a change in conditionOther special projects or functions can be assigned at times by the Director of Nurses and/or the AdministratorEducational/Training RequirementsHas completed an LVN program at an accredited vocational school or college.Licensing RequirementsMust be an active Licensed Vocational Nurse in the state of Texas in good standing.Experience RequirementsExperience working in a skilled nursing facility is preferred.Physical DemandsThe physical demands described here are representative but not necessary all inclusive, of those that must be met by an employee to successfully perform the essential functions of this job.
