Job summary

Val Verde Nursing and Rehabiliation Center is seeking an LVN Charge Nurse to oversee resident care in a skilled nursing setting. Responsibilities include medication administration, rounds with physicians, and ensuring care plans are followed. The role requires TX LVN licensure and prior SNF experience is preferred.

The Charge Nurse will supervise aides, complete required documentation, and coordinate with physicians and family members as needed to ensure high-quality resident care.

Qualifications

• Must be an active Licensed Vocational Nurse in Texas in good standing.
• Experience in a skilled nursing facility preferred.
• Has completed an LVN program at an accredited school.

Responsibilities

• Observe, assess, document, and report resident condition changes during rounds.
• Ensure care plans are implemented as directed.
• Administer medications as prescribed.
• Conduct rounds with physician and record visits in resident records.
• Perform narcotic reconciliation with shifts.
• Identify significant changes and act per facility policies.
• Oversee nursing aides and supervise care delivery.
• Initiate and complete incident/accident reports.
• Discharge residents per facility policy.
• Complete Nursing Assessments before physician calls and communicate updates to families.
