Job summary

The Springs Healthcare & Rehabilitation in Texas is seeking an LVN supervisor for a full-time night shift from 10pm to 6am. This role includes supervising direct care, ensuring safety, and maintaining nursing standards within a SNF setting.

Qualified candidates will hold a Texas LVN license and have prior SNF supervisory experience, with EMR (PCC) familiarity preferred. Competitive benefits and a supportive team are offered.

Qualifications

• Must hold an LVN license in Texas.
• Previous supervisor experience in SNF is highly preferred.
• EMR experience (PCC) is required or preferred.
• Must provide care appropriate to residents' age-related needs.
• Team player with ability to make independent decisions.

Responsibilities

• Follow nursing standards and implement policies and procedures.
• Provide shift handoff reports and ensure follow-up actions are documented.
• Conduct daily resident rounds and report issues to supervisor.
• Identify safety hazards and act to correct them.
• Respond to emergencies per physician orders in absence of physician.
• Supervise direct care associates delivering resident care.
• Monitor narcotic handling and documentation during shifts.
• Notify physicians of changes in conditions or errors.
• Document daily notes for unstable or new residents.
• Ensure resident and staff safety; observe infection control procedures.
• Perform CNA duties when required.
• Other duties as assigned.

What is expected:

• Follow established standards of nursing practices and implement community programs, policies, and procedures.
• Obtain report from nurse being relieved and provide report to nurse coming on duty; record sufficient information to implement appropriate follow-up action, as necessary.
• Conduct resident rounds daily, reporting problems to supervisor and initiating corrective action.
• Identify safety hazards and initiate corrective action immediately.
• Identify resident problems and emergency situations and initiate emergency care per physician’s orders in the absence of a physician.
• Provide supervision to direct care associates providing direct resident care.
• Observe community process related to signing out narcotic and administration thereof. This includes the process related to shift narcotic count and documentation thereof.
• Notify physician, when necessary, of changes in conditions, weight, medication errors, medication not available, abnormal lab/X-Ray results, and any other unusual observations
• Document daily notes on unstable residents/change in resident condition, Medicare residents and recently admitted residents.
• Be responsible for the safety of residents and associates under his/her supervision.
• Observe infection control procedures.
• Perform all the duties of a certified nursing assistant when required.
• Other duties as assigned.

Do I Qualify:

• Unencumbered Licensed Vocational Nurse (LVN) License in Texas
• Previous Supervisor experience in SNF highly preferred
• Electronic Medical Record Experience required, PCC preferred
• Must demonstrate the knowledge and skills necessary to provide care appropriate to the age-related needs of the residents served.
• Be a supportive team member, contribute to and be an example of team work and team concept.
• Must possess the ability to make independent decisions when circumstances warrant such action.
• Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
