Community Options, Inc. in Amarillo, TX is seeking a Licensed Vocational Nurse to provide nursing care in a community setting.

The LVN will deliver care using the nursing process, interact with individuals, families, and health team members, and maintain professional nursing standards. The role requires a nursing license, one year of relevant experience with individuals with disabilities, and a valid driver’s license.

Qualifications

• Bachelor’s degree or equivalent experience in nursing field.
• Minimum of one year of nursing services experience with individuals with disabilities.
• Valid nursing license.
• Valid driver’s license with a satisfactory driving record.
• Ability to calculate figures related to medication dosages.
• Ability to establish a supportive relationship with individuals receiving supports.
• Ability to effectively present facts and recommendations.

Responsibilities

• Provide treatment and health care procedures ordered by a physician and required by professional standards or law.
• Ensure individuals attend medical appointments per their ISP.
• Advise supervisor of concerns related to nursing services.
• Develop and update ISP/PCP with an interdisciplinary team.
• Train employees on medical needs of assigned individuals.
• Monitor health conditions and refer to medical services as needed.
• Monitor medication administration logs and ensure accuracy.
• Maintain program documentation and billable records.
• Build a support network with community members and families.
• Additional tasks and responsibilities may be assigned.
• Cooperate with license and department staff during inspections or inquiries.

We are now hiring a skilledLicensed Vocational NurseinAmarillo,TX.The L.P.N.delivers care to individuals in a community settingutilizingthe nursing process of assessment, planning, intervention, implementation, and evaluation. This person will interact with individuals, significant others, and health team members whilemaintainingthe standards of professional nursing.

Responsibilities

• Provide treatment and health care procedures ordered by a physician and required by the standards of professional practice or law
• Ensure individuals attend medical appointments per their Individual Support Plan (ISP)
• Advisesupervisor of all concerns and issues related to the provision of nursing services
• Develop and update Individual Support Plans (ISP)/Person Centered Plans (PCP) with an interdisciplinary team to include any concerns, observations, and behavioral developments
• Train employees on the medical needs of assigned individuals
• Monitor health and medical conditions of individuals and refer for medical services as needed
• Monitor the use of medications through medication administration logs
• Ensure program documentation and billable records are completed accurately andtimely
• Build a support network for individuals by developing and maintaining relationships with community members, businesses, and families
• Additionaltasks and responsibilities may be assigned
• Employee must cooperate with the license and department staff in any inspection, inquiry, or investigation

MinimumRequirements

• Bachelor’s degree or equivalent experience in nursing field
• Minimum of one year of experience providing nursing services to individuals with disabilities
• Valid nursing license
• Valid driver’s license with a satisfactory driving record
