Job summary

Community Options, Inc. in Austin, TX is seeking a skilled PRN/Contract Licensed Vocational Nurse (LVN) to provide nursing care in a community setting using the nursing process. You will assess, plan, implement, and evaluate care, coordinating with health team members and families.

The LVN will support individuals, maintain licensing standards, and ensure timely documentation while starting pay ranges from $21-23 per hour, depending on experience.

Qualifications

• Bachelor's degree or equivalent nursing experience.
• At least 1 year of nursing experience with individuals with disabilities.
• Valid nursing license.
• Valid driver's license with satisfactory driving record.
• Ability to calculate medication dosages.
• Background checks required (criminal, central registry, child abuse, drug testing).

Responsibilities

• Provide treatment and health care procedures ordered by a physician and required by the standards of professional practice or law.
• Ensure individuals attend medical appointments per their ISP.
• Advise supervisor of all concerns and issues related to nursing services.
• Develop and update ISP/PCP with an interdisciplinary team.
• Train employees on the medical needs of assigned individuals.
• Monitor health and medical conditions and refer for medical services as needed.
• Monitor medication administration logs.
• Ensure program documentation and billable records are completed accurately and timely.
• Build a network with community members, businesses, and families.
• Additional tasks and responsibilities may be assigned.
• Cooperate with license and department staff in inspections or investigations.

We are now hiring a skilled PRN/ContractLicensed Vocational Nurse (LVN)inAustin,TX. The L.V.N. delivers care to individuals in a community setting utilizing the nursing process of assessment, planning, intervention, implementation, and evaluation. This person will interact with individuals, significant others, and health team members while maintaining the standards of professional nursing.

Starting Pay Range: $21-23 per hour depending on experience

Responsibilities

• Ensure individuals attend medical appointments per their Individual Support Plan (ISP)
• Advisesupervisor of all concerns and issues related to the provision of nursing services
• Develop and update Individual Support Plans (ISP)/Person Centered Plans (PCP) with an interdisciplinary team to include any concerns, observations, and behavioral developments
• Train employees on the medical needs of assigned individuals
• Monitor health and medical conditions of individuals and refer for medical services as needed
• Monitor the use of medications through medication administration logs
• Ensure program documentation and billable records are completed accurately andtimely
• Build a support network for individuals by developing andmaintainingrelationships with community members, businesses, and families
• Additionaltasks and responsibilities may be assigned
• Employee must cooperate with the license and department staff in any inspection, inquiry, or investigation

MinimumRequirements

• Bachelor's degree or equivalent experience in nursing field
• Minimum of one year of experience providing nursing services to individuals with disabilities
• Valid nursing license
• Valid driver's license with a satisfactory driving record
• Ability to calculate figures related to medication dosages
• Ability toestablisha comfortable and supportive relationship with an individual receiving supports
• Ability to effectively present facts and recommendations
• Employment is contingent upon successful completion of checks of criminal background, central registry, child abuse registry, and drug testing
