Licensed Vocational Nurse, Home Care Coordinator

Job Overview

Responsible for developing and implementing homecare services for program participants, including coordinating all contracted home services, durable medical equipment, and nursing home care. Attends scheduled IDT and Coordination meetings.

Minimum Requirements

• Graduation from an accredited LVN school and a current LVN license issued by the State of California.
• At least one (1) year of experience practicing as a Licensed Vocational Nurse.
• Knowledge of the physical, mental, and social needs of patients is highly desirable.
• Experience working with a frail or elderly population for at least one year.
• Valid California driver’s license with a good driving record.
• Valid BLS certification or higher, following AHA or Red Cross guidelines.

Compensation

Starting at $31.93 hourly.
