Job summary

Regency Integrated Health Services seeks an LVN Charge Nurse to support Town and Country Nursing and Rehabilitation Center in Boerne, TX. The role focuses on quality resident care, medication administration, and coordination with the care team.

You will observe patients, document changes, and ensure care plans are followed, while supervising aides and reporting to the Director of Nurses and Administrator as needed.

Qualifications

Primary Responsibilities

The LVN Charge Nurse assists in ensuring the provision of quality care that promotes the highest practicable physical, mental and psychological well‑being of each resident, as determined by resident assessments and individual plans of care.

Essential Functions

• Observe, assess, document, and report resident condition and changes during rounds.
• Assure implementation of care plans as directed.
• Administer medications as prescribed.
• Conduct rounds with physician and record visits in clinical records.
• Perform narcotic reconciliation with incoming and outgoing shifts.
• Identify significant changes in resident condition and take necessary action per facility policies and procedures.
• Oversee and monitor aides.
• Initiate and complete Incident and Accident Reports per facility policy.
• Discharge residents per facility policy.
• Complete Nursing Assessment for Physician Calls prior to calling physician on any change in resident condition.
• Communicate with resident’s point of contact when they request a status update or if there is a change in condition.
• Other special projects or functions can be assigned at times by the Director of Nurses and/or the Administrator.

Qualifications

Educational/Training Requirements

• Has completed an LVN program at an accredited vocational school or college.

Licensing Requirements

• Must be an active Licensed Vocational Nurse in the state of Texas in good standing.

Experience Requirements

• Experience working in a skilled nursing facility is preferred.

Physical Demands

• Lifting, carrying, pushing/pulling and sliding/transferring up to 50+ pounds.
• Standing, sitting, walking, speaking, driving, balancing, stooping, kneeling, crouching, crawling, reaching, hearing, seeing, turning, and twisting/leaning.
