Regency Integrated Health Services seeks an LVN Charge Nurse for Town and Country Nursing and Rehabilitation Center in Boerne, TX. The RN/LVN will ensure resident care quality, perform rounds, and administer medications per physician instructions.

The role requires Texas LVN licensure and completion of an LVN program; SNF experience is preferred. This PRN position involves coordinating care and communicating with families and physicians as needed.

Qualifications

Completed an LVN program at an accredited vocational school or college.
Licensing: active Licensed Vocational Nurse in Texas in good standing.
Experience in a skilled nursing facility preferred.

Responsibilities

Observes, assesses, documents, and reports resident condition/changes during rounds.
Ensures implementation of care plans as directed.
Administers medications as prescribed.
Conducts rounds with physician and records visits in clinical records.
Performs narcotic reconciliation with incoming and outgoing shifts.
Identifies significant changes in resident condition and acts per policies.
Oversees and monitors aides.
Initiates and completes Incident/Accident Reports per policy.
Discharges residents per policy.
Completes Nursing Assessment for Physician Calls prior to contacting physician.
Communicates with resident's contact for status updates or changes in condition.
Other duties as assigned by Director of Nurses/Administrator.

Primary Responsibilities

The LVN Charge Nurse will assist in ensuring the provision of quality care that promotes the highest practicable physical, mental and psychological well-being of each resident as determined by resident assessments and individual plans of care.

During rounds observes, assesses, documents, and reports resident condition/changes
Assures implementation of care plans as directed
Administers medications as prescribed
Conducts rounds with physician and records visits in clinical records
Performs narcotic reconciliation with incoming and outgoing shifts
Identifies significant changes in the condition of residents and takes necessary action per facility policies and procedures
Oversees and monitors aides
Initiates and completes Incident and Accident Reports per facility policy
Discharges residents per facility policy
Must complete Nursing Assessment for Physician Calls prior to calling physician on any change in resident condition
Communicates with resident’s point of contact when they ask for a status update or if there is a change in condition
Other special projects or functions may be assigned at times by the Director of Nurses and/or the Administrator

Qualifications

Educational/Training Requirements: Completed an LVN program at an accredited vocational school or college.
Licensing Requirements: Must be an active Licensed Vocational Nurse in the state of Texas in good standing.
Experience Requirements: Experience working in a skilled nursing facility is preferred.
