Quality Talent Group is seeking a Licensed Vocational Nurse (LVN) to join a nonprofit health system with 600+ centers. The role includes reviewing records, scheduling preventive services, and collaborating with providers to ensure continuity of care.

The LVN will educate patients, maintain documentation accuracy, and adhere to clinical and regulatory standards, in a fast-paced, patient-centered environment.

Qualifications

• Graduate of a vocational/technical nursing program.
• Current LVN/LPN license in the state of practice.
• Knowledge of lab procedures, diagnostic testing, and EMR systems.
• Strong documentation, communication, and organizational skills.

Responsibilities

• Review medical records, identify gaps in care, and schedule preventive services per physician-approved protocols.
• Collaborate with providers, Nurse Navigators, and families to ensure continuity of care and patient support.
• Educate patients using evidence-based guidelines to promote engagement and health compliance.
• Maintain documentation accuracy, confidentiality, and compliance with all clinical and regulatory standards.
