Job summary

Quality Talent Group is seeking a Licensed Vocational Nurse (LVN) for a nonprofit health system. The role offers full-time and part-time schedules across day and night shifts, with a focus on patient care and coordination.

Qualifications

• Strong documentation, communication, and organizational skills

Responsibilities

• Review medical records, identify gaps in care, and schedule preventive services per physician-approved protocols.
• Collaborate with providers, Nurse Navigators, and families to ensure continuity of care and patient support.
• Educate patients using evidence-based guidelines to promote engagement and health compliance.
• Maintain documentation accuracy, confidentiality, and compliance with all clinical and regulatory standards
