Job summary

Angels Care Home Health is seeking a Licensed Vocational Nurse (LVN) to provide skilled nursing care in patients' homes under RN supervision. The LVN will coordinate care with the interdisciplinary team, patients and families, and referral sources.

Requirements include graduation from an accredited LVN program, active LVN license in Texas, valid driver's license, CPR certification, and at least two years of LVN experience (home health preferred).

Qualifications

• Graduated from a state-accredited vocational/practical nursing program.
• Current Texas LVN license in good standing.
• Two years LVN experience; home health preferred.
• Valid driver’s license and CPR certification.

Responsibilities

• Administer skilled nursing care in patients' homes under RN supervision.
• Coordinate with the interdisciplinary team and family members to plan care.
• Document care provided and communicate changes to supervising RN.
