Job summary

Angels Care Home Health in Wharton, TX is seeking a dedicated Licensed Vocational Nurse (LVN) to provide skilled nursing care in clients' homes. You will work under the supervision of a registered nurse and coordinate with the care team to deliver compassionate, evidence-based care.

Qualified candidates have a state LVN license, current CPR and Hepatitis profile, a valid driver's license, and at least two years of LVN experience, preferably in home health.

Qualifications

• Graduate of an accredited vocational/practical nursing program.
• Current LVN license in the state.
• Driver's license is required.
• CPR certification and Hepatitis profile required.
• Minimum 2 years LVN experience, home health preferred.

Responsibilities

• Administer skilled nursing care under RN supervision in the client’s home.
• Coordinate care with the interdisciplinary team, patient/family, and referring agency.
