Job summary

Angels Care Home Health is seeking an LVN to provide skilled nursing care in patients' homes under the supervision of a registered nurse. You will coordinate care with the interdisciplinary team, patient and family, and referring agencies.

Required qualifications include graduation from an accredited LVN program, active LVN license, a valid driver’s license, and CPR with Hepatitis profile. Two years of LVN/home health experience is preferred.

Qualifications

• Two years’ LVN experience, home health preferred.
• Active LVN license in the state.
• CPR and Hepatitis profile up to date.

Responsibilities

• Administer skilled nursing care under RN supervision in patients' homes.
• Coordinate care with interdisciplinary team, patients, families, and referral sources.
