Job summary

Quality Talent Group is seeking a Licensed Vocational Nurse (LVN) for a non-profit health system with a broad network of centers. The role supports care coordination, reviews records, and collaborates with physicians, Nurse Navigators, and families to ensure seamless patient care.

The LVN will document accurately, follow regulatory standards, educate patients, and participate in preventive services. The position offers flexible scheduling, comprehensive benefits, and ongoing professional growth

Qualifications

• Graduate of a vocational/technical nursing program.
• Current LVN/LPN license in the state of practice.
• Knowledge of lab procedures, diagnostic testing, and EMR systems.
• Strong documentation, communication, and organizational skills.

• Review medical records, identify gaps in care, and schedule preventive services per physician-approved protocols.
• Collaborate with providers, Nurse Navigators, and families to ensure continuity of care and patient support.
• Educate patients using evidence-based guidelines to promote engagement and health compliance.
• Maintain documentation accuracy, confidentiality, and compliance with all clinical and regulatory standards.
