Job summary

Health Connect America is seeking a licensed-eligible Therapist to provide high-quality mental health services to children, families, and adults. You will conduct biopsychosocial assessments, develop individualized treatment plans, and deliver therapy across individual, group, and family modalities.

You will also document all client interactions per policy, coordinate with a multidisciplinary team, and engage in outreach to promote services and referrals.

Qualifications

• Master’s degree in Social Work, Counseling, or a related field is required.
• Must be eligible for licensure as a LPC, LMFT, or LCSW.

Responsibilities

• Perform comprehensive biopsychosocial assessments to determine clients’ needs and interventions.
• Create individualized treatment plans with individuals, families, and the treatment team.
• Deliver individual, group, and family therapy sessions with measurable goals.
• Accurately document treatment plans, progress notes, and client interactions in a timely manner.
• Collaborate with multidisciplinary teams and participate in treatment team meetings.
