Job summary

Duly Health and Care is seeking an LCPC/LCSW to provide comprehensive counseling services in a dynamic, collaborative setting. You will assess patients, develop care plans, and deliver evidence-based interventions across individual and group formats.

Qualifications include Illinois licensure, a Master's in counseling, CPR, and at least two years of relevant experience in mental health. The role emphasizes teamwork, documentation, and adherence to guidelines to support patient outcomes.

Qualifications

• Valid and Current LCPC/LCSW license issued by Illinois or ability to obtain licensure in Illinois
• CPR certification
• Master's degree in counseling
• 2 years of experience
• Experience conducting individual therapy sessions
• Experience with clients with diverse mental health needs
• Experience in a multidisciplinary team (preferred)
• Experience with group therapy (preferred)
• Knowledge of current assessment protocols
• Knowledge of care plan development
• Knowledge of mental health disorders/DSM-5
• Excellent listening and interpersonal skills
• Excellent written and verbal communication skills
• Strong critical thinking and analysis skills
• Proficient with computers and Microsoft applications

• Ability to perform psychosocial assessments and develop viable care plans

Responsibilities

• Provide support and guidance to help patients address personal challenges
• Conduct initial assessments to determine needs and therapy goals
• Develop and implement care plans based on assessment data
• Evaluate and diagnose psychological concerns per DSM-5
• Meet with patients regularly to discuss problems, feelings, and relationships
• Help patients explore thoughts, behaviors, and past experiences
• Plan and implement empirically validated interventions
• Communicate complex information to patients and families
• Refer patients to other disciplines as needed for consultations
• Maintain current clinical records and timely SOAP notes
• Comply with regulations and guidelines
• Document patient data accurately and timely
• Administer self-report scales (PHQ-8/PHQ-9, etc.)
• Provide evidence-based treatment and collaborate with colleagues
• Attend supervision and meetings on best practices
• Complete mandatory organizational training
• Review literature and translate behavioral health concerns into practice
• Serve as a resource to other disciplines
• Provide psychological education to multidisciplinary teams
• Comply with state licensure requirements
