Job summary

Duly Health and Care seeks a licensed LCPC/LCSW with a Master’s in counseling to provide individual psychotherapy and psychosocial assessments in a dynamic multidisciplinary setting in Illinois. You will develop care plans, document clinical notes, and collaborate with a diverse team to support patient outcomes.

Qualifications

• Master's degree in counseling required.
• Experience providing individual psychotherapy.
• Illinois licensure (LCPC/LCSW) or ability to obtain it.

Responsibilities

• Conduct initial assessments to determine patient needs and goals for therapy.
• Develop and implement individualized care plans based on DSM-5 aligned diagnoses.
• Meet with patients regularly to discuss problems, feelings, thoughts, and relationships.
• Document SOAP notes timely and accurately; maintain active clinical records.
• Provide evidence-based psychotherapy and collaborate with multidisciplinary teams.
• Attend supervision and trainings to stay current with best practices.
• Comply with licensing, organizational, state, and federal regulations.
