Eventus WholeHealth seeks a licensed psychotherapist (LCSW/LISW/LMFT/LPCC/LCMHC) to join our team serving residents in skilled nursing and assisted living facilities. You will provide recovery-oriented rehabilitation services in group or individual settings, across mental health issues, within a multidisciplinary team.

You will document treatment plans, collaborate with primary care physicians and facility staff, ensure HIPAA compliance, and follow Eventus policies.

Qualifications

• Master’s degree in social work or equivalent.
• Current license to practice in the state (LCSW/LISW/LPCC/LCMHC/LMFT).
• Experience with assessment and psychotherapy within healthcare settings.

Responsabilités

• Assess residents and develop individualized treatment plans.
• Coordinate with residents’ primary care physicians, family, and facility staff.
• Document treatment plans, sessions, and updates per policies; maintain medical records.
• Maintain open communication with facility staff and administration.
• Adhere to all regulations and HIPAA confidentiality requirements.
• Consult with clinical supervisor when facing unfamiliar requests or cases.
