Job summary

Health Connect America is seeking a Licensed Therapist to deliver evidence-based mental health services across homes, communities, and offices. You’ll conduct thorough assessments, create customized treatment plans, and collaborate within a multidisciplinary team to support children, families, and adults.

The role requires a Master’s in Social Work, Counseling, or related field and active Ohio licenses (LSW/LISW/LPC/LMFT/LPCC).

Qualifications

• Master’s degree in social work, counseling, or related field required.
• Must hold Ohio licenses: LSW, LISW, LPC, LMFT, or LPCC.
• Experience in clinical assessment and treatment planning preferred.

Responsibilities

• Perform biopsychosocial assessments to determine needs and interventions.
• Develop and implement individualized treatment plans with clients and families.
• Deliver therapy in individual, group, and family sessions; document progress.
• Ensure accurate documentation and regulatory compliance across caseloads.
• Provide crisis intervention and safety planning as needed.
