Job summary

Avalon Rehabilitation and Healthcare Center is seeking a licensed Social Worker to support residents and families with psychosocial assessment, care planning, and coordination within a 180-bed sub-acute and long-term care setting.

The role requires NJ LSW or CSW licensure, a bachelor’s degree with master’s preferred, and SNF/LTC experience. You will collaborate with interdisciplinary teams, provide counseling, and ensure compliance with facility policies while promoting quality of life.

Qualifications

• Must have NJ LSW license or CSW.
• Bachelor’s degree required; Master’s degree strongly preferred.
• Experience as a Social Worker in healthcare, SNF/LTC preferred.
• Excellent communication and interpersonal skills.
• Ability to work in a fast-paced, team-oriented environment.

Responsibilities

• Evaluate residents to assess psychosocial needs and outcomes.
• Address resident goals and emotional needs with families and case managers.
• Coordinate care with interdisciplinary teams.
• Develop, implement and update psychosocial care plans.
• Participate in care planning meetings and ensure continuity of care.
• Provide counseling and resource referrals.
• Ensure compliance with facility policies and documentation standards.
