Avalon Rehabilitation and Healthcare Center, a 180-bed Sub-Acute and Long-Term Care facility in Hamilton, NJ, is seeking a licensed Social Worker. You will assess residents' psychosocial needs, develop care plans, coordinate services, and support families to promote well-being and successful outcomes.
Requirements include NJ LSW or CSW licensure, a Bachelor’s degree (Master’s preferred), and experience in healthcare settings, especially SNF/LTC.
