• Identify medically-related social services needs and ensure provision in line with regulations.
• Document social service actions and maintain compliant records in residents' medical files.
• Engage residents and families in care plan development and advance care planning discussions.
• Coordinate preadmission services, discharge planning, and community resource referrals.
• Advocate for residents' rights and report any abuse or noncompliance following policy.
